Heath Service Navigator Privacy Policy

This website located at www.healthservicenavigator.com (“Site”), is used by The Manufacturers Life Insurance Company (“Manulife”) for the purpose of providing Health Service Navigator (“HSN”), and is maintained and operated by Health Care Services International Inc. (“HCSI”), which carries on business as Novus Health. Manulife, HCSI and their respective affiliates (collectively referred to as the “Companies”, each a “Company”) are all committed to protecting your privacy. This privacy policy (“HSN Privacy Policy”) applies to the collection, use and disclosure by any or all of Manulife, HCSI and their respective affiliates of information about identifiable individuals (“Personal Information”) collected through or in relation to their use of this Site. However, as Manulife is using this Site to provide HSN, you should review the privacy policy applicable to the use of Manulife’s Canadian division website (“Manulife Privacy Policy”) which is located at http://www.manulife.ca/, as it may also apply. Nothing in the HSN Privacy Policy is intended to derogate from the Manulife Privacy Policy.

1. Collection of your Personal Information

Manulife may collect Personal Information such as your name, email address, contact name, company name, address and telephone number, your group/plan number and member/certificate number, and such other information that is necessary so that you may be assigned identifiers to enable you to access the Site (the “Registration Information”). Personal Information will also be collected by either or both of HCSI or Manulife when you voluntarily choose to supply it through the Site.

HCSI may collect information about your visit to the Site, including the pages you view and the links you click. HCSI also collects certain standard information that your browser sends to every web site you visit, such as your IP address, browser type and language, access times and referring web site addresses. This information will not be shared by HCSI with Manulife or Manulife’s affiliates, unless it is provided to Manulife in an aggregate format, from which individuals cannot be identified.

If you choose to submit any personal health information on the Site, it will only be collected temporarily, for the purpose of administering any health questionnaires or tests which you have selected. However, upon either logging off of the Site or closing your web browser, the information will not be retained.

2. Use of your Personal Information

Manulife and HCSI use the Personal Information you provide through http://www.manulife.ca/ (the “Manulife Website”) or this Site to create a user account so you can access this Site. Your Personal Information, collected through this Site is also used to assist the Companies in delivering customized services and products to better meet your needs.

HCSI may use your Personal Information to verify that you are authorized to use the Site. HCSI may also use your Personal Information to generate statistics and reports for internal use, and to evaluate the performance and usage frequency of the Site, in an aggregate format, from which individuals cannot be identified.

3. Disclosure/Transfer of Your Personal Information

Manulife may disclose the Registration Information to another Company. The Companies will not sell, rent or lease your Personal Information to any third parties. The Companies may access, use and/or disclose your Personal Information if required or permitted to do so by law. A Company may also access, use and/or disclose your Personal Information under the following circumstances:

1. to comply with a legal requirement including but not limited to one imposed by a warrant, subpoena, court order or like instrument served on any Company;
2. where a Company has reasonable grounds to believe that such access or disclosure is necessary to prevent or investigate any breach of an agreement or contravention of a law; or
3. in urgent circumstances to protect the life, health or security of any person.

Other than as set forth above, and subject to any greater use or disclosure to which you may have agreed under the terms and conditions of the Manulife Website,

1. a Company will not use or disclose your Personal Information without your consent; and
2. any use or disclosure of your Personal Information within or between Companies will remain subject to the limitations of this Privacy Policy; and
3. each Company will give you notice and obtain your consent before such Company uses or discloses your Personal Information for any purpose that is not described in this Privacy Policy.

Each Company may also disclose your Personal Information to its successor or any assignee, as necessary, without notice to you.

4. Security of your Personal Information

The Companies are committed to protecting the security of your Personal Information. Each Company uses a variety of security and information technologies and procedures to help protect your Personal Information from unauthorized access, use, or disclosure.

5. Use of Cookies

HCSI uses “cookies” to facilitate your online experience and, in some instances, to provide you with access to the Site. A cookie is a text file that is placed on your hard disk by a web page server. Cookies cannot be used to run programs or deliver viruses to your computer. Cookies are uniquely assigned to you, and can only be read by a web server in the domain that issued the cookie to you. One of the primary purposes of cookies is to ensure that you are authorized to use the Site. Some of the cookies used by HCSI are encrypted cookies which contain your insurance contract and certificate number. Where these cookies are used, they are used for the purposes of authenticating and providing you with access to the Site.

You have the ability to accept or decline cookies. Most Web browsers automatically accept cookies, but you can usually modify your browser setting to decline cookies if you prefer. Please note that if you choose to decline cookies, you may not be able to access or use some or all of the content on the Site.

6. Aggregate Data

HCSI may provide Aggregate Data to third parties. “Aggregate Data” means similar pieces of non-personal information (i.e., information that cannot be used to identify a particular individual) that are gathered by the Site from two or more users of the Site and are combined.

7. Changes to this Policy

The Companies encourage you to periodically review this HSN Privacy Policy and the Manulife Privacy Policy. This HSN Privacy Policy may occasionally be updated or changed. When changes are made to this HSN Privacy Policy, you will see a message on the introductory page of the Site indicating an update has been made to this HSN Privacy Policy. Any such message will be posted for a period of at least thirty (30) days from such update. The date that this HSN Privacy Policy was last updated will also appear at the top of the HSN Privacy Policy.

8. Enforcement of this Privacy Policy

If you need additional information about a Company’s privacy policy or its management of your Personal Information, or if you want to access the Personal Information that a Company has collected about you, or if you want to inform a Company of any information it has on file that you believe to be incorrect or incomplete, please contact the applicable Company.
