Service limitations
Some of your dental coverage may be limited to a number of treatments or time limitations. Check below to find out how frequently you're covered for each treatment, or whether any other limitations apply.

A pre-determination of benefits must be submitted for written approval of any major services such as crowns, dentures, or bridgework. X-rays may also be required. *Coverage for dentures and bridgework may be subject to the open space limitation. This means that your plan may not cover dentures or bridgework required to replace a natural tooth which was missing before the date you became covered. If a crown is being replaced within 5 years of the previous crown, then the new crown will be reimbursed at a reduced rate. |
