1.  The personal information you have submitted and will 
    continue to submit is your own and you are only using 
    the group/plan number, plan member/certificate number 
    and activation key* that has been assigned to you 
    personally by Manulife Financial.
2.  You are not registering to use the site on someone 
    else's behalf.
3.  You will keep your activation key and password 
    confidential at all times and you understand you 
    are responsible for all user transactions 
    performed on the site using your activation key 
    password.
4.  You will notify Manulife Financial Group Benefits by 
    telephone at 1-800-268-6195 (former Maritime Life 
    customers call 1-866-507-2727)  within 24 
    hours of learning that your activation key and 
    password were disclosed to or obtained by anyone else.
5.  You agree to uphold these and all obligations set out 
    in the Terms and Conditions and you understand that 
    Manulife Financial has the right to withdraw your 
    access to the site immediately, in the event that any 
    of the obligations set out in the Terms and Conditions 
    have been broken.

*Activation key only applies to new Plan Member Site registrants after February 16, 2003.

    TERMS & CONDITIONS FOR USE OF THE SITE

On this web site, the terms "you," "your" and "the user" refer to you, the plan member of a Manulife Financial Group Benefits plan with access to this site; and "Manulife Financial", "we," "our" refers to Manulife Financial Group Benefits, a division of The Manufacturers Life Insurance Company.

In order to access and use the Manulife Group Benefits secure Internet site, you must agree to the following terms and conditions. All terms and conditions bind you, your heirs, executors, liquidators, administrators and assigns.

1. Use of Manulife Financial Group Benefits Plan Member Site

The information and contents of the Site are provided strictly for your convenience. This Site is located in Ontario, Canada. You agree to comply with all of the Terms and Conditions set out in this document.

2.  Your personal site identification codes
-------------------------------------------------------------
In terms of the Manulife Financial Group Benefits Plan member secure site, your personal site identification codes which are used to access this site include:
i)   Your plan member/certificate number assigned to you 
     by Manulife Financial;
ii)  Your group/plan number assigned by Manulife 
     Financial; 
iii) Your activation key assigned by Manulife 
     Financial; and 
iv)  A password, selected by you.

You agree to comply with all written Manulife Financial instructions provided with respect to your identification codes, as well as with the following:
i)   to keep your activation key and password confidential
     at all times;
ii)  to apply the highest level of care in using the 
     identification codes to avoid theft, misappropriation,
     unauthorized access and unauthorized communication 
     and agree that this is a reasonable request;
iii) to notify Manulife Financial Group Benefits by 
     telephone at 1-800-268-6195 (former Maritime Life 
     customers call 1-866-507-2727)  within 24 
     hours of learning that your activation key or password 
     were disclosed to or obtained by anyone else or may be 
     known by anyone else, or that unauthorized use of 
     the Manulife Financial Group Benefits Plan Member
     Site may be occurring. 

The use of your activation key and password by you, or by any person with or without your knowledge or consent binds you legally unless you have previously notified Manulife Financial Group Benefits that the confidentiality of your identification codes has been compromised.

3.  Communications by electronic means

In addition to communications by voice and mail, you authorize Manulife Financial Group Benefits to communicate with you by fax, or by online notices or electronic mail to your personal computer, at such numbers or addresses as you provide to us. We will not be responsible or liable for any loss or damages you may incur if you communicate confidential or other personal information to Manulife Financial Group Benefits by fax, or by other electronic means from a personal computer including electronic mail, or if Manulife Financial Group Benefits communicates such information to you by these means at your request or as a reply to a communication sent by you over the same channel.

4.  Verification of communications

We may verify communications, or the source of the communications before we accept them, but we are not obligated to do so.

5.  Exclusions

You agree and acknowledge that complete privacy, confidentiality and security is not yet possible over the Internet, and that since the Internet is not a fully secure method for the communication of information, and that your privacy and confidentiality cannot be guaranteed Manulife Financial shall not be responsible or liable for any harm or loss that you, or any other person, may suffer in any connection with your access to, or use of, the site.

Manulife Financial believes that the information included on the site is accurate when it is posted on the site; however, such information is subject to change and update at any time without prior notice. You understand and agree that Manulife Financial can not, and does not, guarantee nor ensure either the accuracy, completeness, nor authenticity of the site's contents, site functionality, or your transmission of any information or instructions to, or from, the site. You agree that neither Manulife Financial, nor any of its affiliated companies, nor any other person associated with the creation or use of the site, shall be liable or responsible to any person for any harm, loss or damage whatsoever that may arise in any connection with your use of the site.

You waive all rights that you may have to hold Manulife Financial responsible in connection with any other use of the site.

Based on the emerging nature of Internet transactions and related technology, you agree that these exclusions of liability are fair and reasonable, and that Manulife Financial would not have permitted use of the site by you if not for your agreement with the liability exclusions and disclaimers set out herein.
 
You understand that the availability of this site to you is governed in part by your employer's contractual arrangements with Manulife Financial and your continuing membership in the group benefits plan(s), which may change from time to time.
You understand that any such changes in availability shall be communicated to you by your employer and Manulife Financial assumes no liability for such communication or lack thereof. 

Manulife Financial Group Benefits Plan member secure site is not intended to be a comprehensive explanation of your group benefits. You are to refer to your Benefit Booklet for more information, or contact your plan administrator or the person responsible for administering your benefits plan, to resolve any questions you may have requiring a detailed description of the benefits covered under your plan.

In the role of administering your benefits plan, your plan administrator may access your information on this site.

6.  Change of these terms and conditions

Manulife Financial may change these terms and conditions and the services provided by this site from time to time. You will be required to agree to accept these changes on the effective date in order to continue using this Site. 

Manulife Financial has the right to rescind or discontinue your access to this site at any time without explanation to you.
