Submit a termination request

When a member leaves your plan, you can notify Manulife Financial of the termination using this feature. Remember to also submit the last contribution for the terminating member.

Transactions entered after 4:00pm EST will be processed on next business day.

Note - If termination is due to death, complete a Notice of Death form (GP0770E) and return the completed form to Manulife Financial with the appropriate proof of death.
