If a member need to change coverage for a reason not listed above, please contact customer service.
A waiting period may have to be satisfied before a plan member is eligible for a coverage change if the life event is due to a new common-law relationship or a separation. The "Date the selected event occurred" should be the date that the waiting period was satisfied.
If the life event is due to a marriage or divorce then there is no waiting period.
