The information gathered will be reviewed to determine eligibility and that review cannot be completed until all of the information has been received. In some cases, it may be necessary to gather additional information before a decision can be made. We will notify you if this becomes necessary.
Instructions for submitting online forms
Making an online submission at Manulife is quick, easy and confidential. All you have to do is follow a few steps:
Step 1: Select the benefit for which you are applying. Please note that if applying for Long-Term Disability (LTD) benefits, the application for Critical Illness, Life Waiver and Accidental Death & Dismemberment are included on the LTD form.
Step 2: Fill in the online form. You will see some of the information is pre-populated for you. You can also attach documents to your submission (e.g. a copy of job demands). If you cannot complete the form in one sitting, you can always save it and come back later.
Step 3: Review if there are any additional documents necessary, and upload them to your submission.
You can make edits to your submission at any time prior to submitting the form online. Once submitted, you will not be able to make changes.
