Expenses eligible for coverage under any type of government-sponsored plan or workers� compensation board
Lenses prescribed for the following conditions:
- Severe corneal astigmatism
- Keratoconus
- Aphakia

For these types of vision claims, submit a paper claim form .

Online claims submissions must be performed by a registered service professional. If your receipt does not have a designation and registration number, you must submit your claim using a paper form.
