Instructions

To transfer assets from amongst your current investment(s) to other funds available to you under the plan:

1. Specify either the dollar ($) amount or percentage (%) of the investments from which you wish to transfer from.
2. Transfer these amounts ($) or percentages (%) to the appropriate target funds.
3. Submit these changes, confirm and print reference page.

Transfers can include both dollar amounts and percentages.
Dollar amounts transferred cannot exceed the 90% maximum.
To transfer all of the assets from a fund, simply indicate 100% in the Percentage (%) column.
